The scout and the Heavy cannot read German, so when they happened upon the medic's diary, they immediately employed the spy for his translating skills.
For all you fanbrats out there - I have usually heard the term "dark eyes" as a generic description of Russian loveliness. I know that the heavy has blue eyes - the medic is exaggerating for the purpose of poetic license because he is a FAGGOT. GAWD - i feel old.
Artistic nitpick: I realize this picture is almost 2 years old, but you asked for feedback.
Something bothers me about the Heavy's eye region. Not only do his cartoonishly round eyes look odd compared to the other two characters' eyes (I realize this is supposed to show surprise but it doesn't IMO fit the more realistic art style), his eyebrows are black (and thus prominent) and very close together over the root of the nose, and his forehead looks extremely low here. Looking at the actual character model, his eyebrows are more arched, brown rather than black, and space much wider apart. His forehead isn't that low, either.

I'm pretty sure the Spy knows German. He's French, he's a Spy, France and Germany have had 3 wars since 1870 alone. The two countries used to be considered "arch-enemies", much like the French and the English for centuries.
Question is, regardless of what Spy claims he can or cannot do, or knows or does not know... do you actually believe hims words? He's Spy! Lying is part of his profession. He even lies to himself, I am sure.
So it's even questionable if Spy's translation is indeed correct or just made up to mess with Heavy's head. I mean, the good Doctor seems far more into blood and ethically dubious experiments involving xenotransplants and heavily muscled, protective Russian bears than babies.
Unless... the Medic got it into his Mad-Science-addled brain that the best method to breed the perfect specimen, "Ze Übermensch", was to create a child from his own DNA (of course! who else!) and his beloved Heavy's seed. Oh dear. Kill me now.

Other than that, I love the expressions of the characters here, and the colours.
